Exercise Therapy QLD provides Accredited Exercise Physiology (AEP) for patients referred by their GP under a Medicare Chronic Disease Management (CDM) Plan, alongside DVA, private health insurance, and NDIS clients. Sessions are evidence-based and tailored to help you manage your condition, build strength, and improve function over time.

How Medicare CDM Funding Works
A Chronic Disease Management referral is arranged by your GP through a GP Management Plan and Team Care Arrangement, for conditions that have been (or are likely to be) present for six months or longer. Medicare provides a rebate towards up to 5 allied health sessions per calendar year, shared across all allied health providers you see under the plan, not exercise physiology alone. A gap payment may apply depending on the session fee — your GP or our team can talk you through the current Medicare Benefits Schedule rebate amount.

Bring your GP Management Plan and Team Care Arrangement referral to your first appointment, and we’ll take care of the rest — including sending a written report back to your GP at the end of your referral cycle.
